| 内容記述 | 脳卒中患者の在宅移行期の看護ニーズを探る目的で，退院後在宅療養し研究参加に同意の得られた患者４名を対象に調査を行った．調査期間は2004年12月より2005年12月で，退院後1ヶ月，２ヶ月，３ヶ月の 各時期に，脳卒中が原因と考えられるからだの状態と日常生活動作の困難の程度，心理状態に関する質問紙を用いて面接調査を行った．結果，患者は身体機能の低下と頭痛や倦怠感，疲労感といった全身症状を知覚し，そのことで日常生活動作に困難を感じていた．身体状態の知覚や日常生活での困難感は各時期を通して大きな変化は見られなかった．また， 各時期を通して軽度抑うつ的な状態が見られた．身体状態や日常生活動作の困難に対して，社会資源活用，自助具の活用，リハビリテーション，環境の改善などの対処をおこなっていた．このことから，在宅移行期にある脳卒中者は身体機能の維持向上や四肢の感覚異常の改善，全身症状の軽減への看護ニード，抑うつ状態の改善，障害を持った自身の身体を受け入れ新たな日常生活を再構築するための看護ニードを有していることが示唆され， 抑うつ的な状態への援助と共に，日常生活の確立，全身症状への対処方法の獲得，身体機能の維持向上にむけた援助が必要と考えられた． | |||||||||

| 内容記述 | This study was performed to obtain suggestions for nursing needs to patients with cerebrovascular disease in their transition from hospital to home. A survey was conducted on 4 patients who were recuperating at home after hospital discharge and who provided consent to participate in this study. The survey period was December 2004 to December 2005. A questionnaire was used to determine the physical symptom and degrees of difficulties in activities of daily living which were considered to arise from the cerebrovascular disease and CES-D. An interview survey was conducted. The Questionnaire and interview survey were performed 1, 2, and 3 months after patients were discharged from the hospital. The results indicated that the patients perceived a decline in their physical functions and systemic symptoms at each surveyed time, and they felt difficulties in activities of daily living. They managed systemic symptoms and decline in their physical functions, by use of social resources, rehabilitation, and environment etc. They had a tendency to depression state at each surveyed time. From the findings of this study, the needs were their considered for cerebrovascular disease patients in their transition from hospital to home: establishment of daily life, acquisition of a management method for systemic symptoms, improvement of a tendency to depression state, and assistance for maintaining and improving physical functions. | |||||||||
