| 内容記述 | 人工肛門造設術を受ける患者は，医師から大腸がんと診断され人工肛門造設を告知されることによっ て，ボディイメージの変化・喪失という予期せぬ困難に直面する．本研究の目的は．人工肛門造設を告知された患者が診断を受けてから手術目的で入院するまでに，どのような体験をしたのか理解することであり，人工肛門造設を控えた患者に対する看護援助のあり方を検討する際の一助とすることである．人工肛門造設を告知された患者3名の語りから，病名や治療方法を知らされた患者は，自分の病気を認識するとともに，自らの死を意識するということがわかった．また，患者は人工肛門に対する否定的なイメージによって大きな衝撃を受け，今後の生活に対する不安を抱いていた．しかし一方で，家族に支えられていることを実感すると患者の気持ちは前向きに変化していくことが理解できた．そこで看護師は，精神的な危機状況に置かれた患者の内面を理解しながら， 患者のみならず家族に対しても術前から人工肛門に関する正しい知識を伝え，不安の軽減に努めることが重要である． | |||||||||

| 内容記述 | When patients with ａ colostomy were first informed by their doctor that they had ａ colon cancer and ａ colostomy should be installed, they faced unexpected hardships of change and loss of their body image. In order to examine nursing care for patients who are waiting for their operation of colostomy, this study focuses on understanding of patient experiences between diagnosis and hospitalization for an operation. By means of analysis and interpretation of narratives from three such patients, we understood that, after they were informed of their illness and medical treatment they would become conscious not only of their illness but also of their death itself. In addition, those patients got shocked on account of the negative images of colostomy and anxious about their future life. On the other side, it was also made clear that they would act well and live positive as they felt they were supported by their family. In conclusion, it is vitally important for nurses to understand that patients with ａ colostomy should face severe mental health crisis, provide accurate information prior to surgery for patients and their family about colostomy, and try to alleviate their anxieties. | |||||||||
