| 内容記述 | 腹膜透析を続ける患者の中には，腹膜の機能低下や感染等の理由によって，血液透析への移行を余儀 なくされる患者が少なくない．本研究では，腹膜透析から血液透析への移行を決断した患者の病気や治療をめぐる体験を理解し，末期腎不全患者に対する看護援助のあり方を検討した．血液透析移行後の患者3名の語りから， 治療方法を変更せざるを得ない状況は，患者にとって身体的にも精神的にも危機的な状況であり，生きていくための選択と決断に直面するということが理解できた．また，患者は前向きに生きていこうと思いながらも，さま ざまな葛藤を抱いていることがわかった．そこで看護師は，生涯にわたって透析治療が必要となる患者に対して， 心身の危機的状況が訪れることを治療開始時から予測してかかわる必要がある．それによって，患者は疾患の経過や治療に伴う生活の変化を踏まえながら，人生の行く末を見つめ自らの将来像を描けるようになり，危機的状況に備えることができると考える． | |||||||

| 内容記述 | Not a few patients receiving continuous ambulatory peritoneal dialysis have no choice but to determine shift to hemodialysis treatment on account of their function decrease of peritoneum and infection. The purpose of this study is 1) to understand experiences of patients who determined shift from continuous ambulatory peritoneal dialysis to hemodialysis and 2) to examine nursing care for terminally ill patients with chronic renal failure. By examining narratives of three patients experiencing this treatment shift, we understood that this shift 1) gives patients physical and psychological burdens and 2) forces patients to determine a new life-style with the disease. In addition, we understood that patients suffer mental conflicts even though they try to face and deal with their difficult situation in an active and positive manner. Then we came to a conclusion that nurses should anticipate physical and mental crises of patients in need of lifelong hemodialysis treatment at the beginning. Patients can face their present situation, consider their future life-plan, and prepare for crises with certain knowledge of their actual course of the disease and the possible changes in their life associated with the treatment shift. | |||||||
