| 内容記述 | A地域唯一のがん診療連携拠点病院の病棟看護師と同じ二次医療圏で活動する訪問看護師の地域緩和ケアに関する実践と認識を明らかにし,切れ目のない緩和ケアを提供するための課題を検討することを目的に,自記式質問紙調査を実施した.訪問看護師28名,病棟看護師47名から回答が得られた.訪問看護師は本人と家族に対するコミュニケーションと患者・家族中心のケア,病棟看護師は疼痛と呼吸困難のケアを実施している認識が有意に高いことが示された.がん診療連携拠点病院と訪問看護の連携については,自由記述から両者どちらも連携を図ることを課題として認識していることが明らかになった.A地域においては今後,患者・家族を中心としたケアの実践や両者の連携の在り方を具体的に検討する必要性が考えられた.さらに,訪問看護師と医師との相談や協議をしやすい体制の構築,病棟看護師自身が在宅緩和ケアを知る機会を増やすとり組み,訪問看護師が病棟看護師にフィードバックする実践の必要性が示唆された.(著者抄録) | |||||

| 内容記述 | This study aims to describe the palliative care and awareness of visiting nurses working in the same secondary medical area with ward nurses of the solitary designated cancer hospital in region A, and also to examine issues for providing seamless palliative care. For these purposes a self-rating questionnaire survey was conducted, and responses from 28 visiting nurses and 47 ward nurses were collected. Visiting and ward nurses showed a very strong awareness of conducting the following activities: communication with patients and families, patient/ family-centered care (visiting nurses); and pain and dyspnea care (ward nurses). In regard to the designated cancer hospital’s cooperation with visiting nursing, from self-rating questionnaire survey, the both types of nurses recognized the cooperation as issues. In region A, it is necessary to discuss specifically the practice of patient/ family-centered care and the way of cooperating with each other. The findings suggest the necessity to establish an environment where visiting nurses can consult and discuss with physicians, to increase opportunities for ward nurses to learn about home palliative care, and for the visiting nurses to provide feedback to ward nurses. | |||||
