| 内容記述 | 本研究の目的は，がん患者の地域緩和ケアに関する認識を明らかにすることである．その上で地域 緩和ケアの充実を図る上での課題を検討した．Aがん診療連携拠点病院の緩和ケア外来に通院する患者30名に対して，地域緩和ケアに関する認識について質問紙調査を実施し，18名から回答を得た．回答者は49歳から92 歳まで（平均年齢：67.1歳）の男性15名，女性3名であった．回答者の83.3%は病気に伴う心の悩みがあり， 72.2%は身体的な痛みを抱えていた．回答者の11.1 % は社会復帰や家族の健康や介護についての悩みもあることが明らかになった．また66.7%の回答者はAがん診療連携拠点病院が地域で機能していると認識していたが，回答者の72.2%は，自宅における医療や看護ができていないと認識していた．自由記述では，緩和ケアの存在を早期に知ることを希望していた．これらのことから，緩和ケア外来を利用する患者だけでなく家族も利用できる相談の場と機会を増やす必要性があるという課題が確認された．心身の苦痛に対する積極的な介入が重要であると考えた．また患者が退院後に在宅で安心して生活するためには，関係者の情報共有と共に，入院中から緩和ケア 外来，在宅医療，訪問看護とのつながりが必要である．地域医療従事者が緩和ケアに加わることによって，退院後の療養生活で生じる不安を軽減できると考えられる．加えて，地域住民への地域緩和ケアに関する情報発信が必要である． | |||||

| 内容記述 | The purpose of this study was to clarify the perceptions of cancer patients about regional palliative care. Moreover, we considered the problems with the improvement of the regional palliative care. The participants were 30 outpatients at the palliative care clinic in the A designated regional cancer care hospital. A questionnaire survey of the perceptions about regional palliative care was conducted. The questionnaires were collected by mail from 18 participants (15 males, 3 females). The range of the participants’ age was 49 to 92 (mean: 67.1). The results showed that 83.3% of the respondents experienced the mental distress owing to the disease, 72.2% suffered from physical pain, and 11.1% felt uneasiness over the return to society and the distress concerning family members’ health and caregiving. Also, 66.7% of the respondents thought that A designated regional cancer care hospital is functional in the region. However, 72.2% of them thought that medical and nursing care was not conducted at home. A few participants wanted to know earlier about palliative care. We identified the necessary for the intervention to increase places and opportunities of the consultation for outpatients and their families. And we found it important to intervene actively in the mental and physical distress. To live with peace of mind at home after hospital discharge, they also need to have connections with a palliative care clinic, home medical care and visiting nurses in the hospital as well as share information with the people concerned. Community health care providers’ participation in the regional palliative care team could reduce discharged patients’ anxiety at home care settings. In addition, we need to send information about regional palliative care for residents. | |||||
