本研究は医師のがん告知に関する考え方と，現状及び患者が理解した告知の内容について比較検討 した．また患者は外来受診から，入院，退院を通した医療者との関わりをどのように受け取っているのかについて明らかにした．対象者はＡ病院の外科医師６名とがんの手術を受け入院を経験した外来通院患者２６名である． 同意を得た後，半構成的面接を行い，面接で得られた内容について内容分析を行った．
医師はほぼ患者全員に告知を行っているとしていた．患者の受けた告知内容は，「がん」と病名を告げられた， それとなくほのめかされた，治療の説明を受けた（手術を勧められた），知らされなかった，の４つに分類された． 治療方法を説明された患者よりも病名を告知された患者，ほのめかされた患者の方が医師の説明を「分かった・ 理解できた」と評価していた．医療者の関わりのうち看護婦に関連した評価は「親切」「やさしい」等であり具体的な内容はほとんどなかった． |

The purposes of this research are 1）to examine, by a comparative method, how doctors inform cancer patients of the diagnosis and how the patients understand the doctors’ information, and 2）to clarify the patients’ thoughts and feelings toward the health professionals. The setting for this study was a public hospital in N prefecture. The subjects were six surgeons and ２６ outpatients who received an operation for cancer. In the semi-structured interview, the doctors were asked how they told the diagnosis to patients, and the outpatients were asked how they recognized and felt the doctors’ words. The doctors reported that they told diagnosis to the patients. However, content analysis of the patients’ interview data revealed the doctors’ four different styles of cancer telling: 1）using the word of“ cancer” , 2）vague wording, 3）description of the treatment (operation） and 4）telling nothing about diagnoses. The patients who were given the word of“ cancer” or vague wording could understand the meaning of the doctor’ s information better than the patients who were informed of the treatment. The patients considered nurses as“ kind”“, attentive” or“ sympathetic” and did not mention their specific behavior or attitudes. And the doctors expected nurses to carry out their task with professional competence. |
