本研究は,小児科外来看護師が慢性疾患をもつ子どもの家族へどのようにかかわっているのかを知り， 家族とのパートナーシップ形成や専門職者間の連携について，Ａ県内の50人の小児科外来看護師から得た質問紙 調査結果を元に検討した． その結果，外来看護師は，家族に指導や説明をしても手ごたえがないことや拒否的な態度によりかかわりがもてないことなどに戸惑いや困難を感じていた．その中には，看護師が時間をかけてかかわることで家族との関係 が変化したり，専門職者とともに家族へかかわっていた事例もあったが，専門職者との連携が難しい状況もみられた． 多くの外来看護師は，家族とパートナーシップを形成するための働きかけや，専門職者との連携・協働について，重要であると認識していたが，現状で行えているとはいえなかった．また，小児科専任でない看護師は専任 の看護師と比べて，子どもの看護に必要な知識や技術不足を感じていたことも明らかになった．外来看護師が専門性を持てるようなシステムの改善とともに看護師を支援するための教育的取り組みも必要であることが示唆さ れた。 |

The purposes of the present study were to explain how outpatient clinic nurses assist family members in providing care for chronically ill children and to investigate partnerships between nurses and family members and collaborative care among medical professionals. Subjects were 50 outpatient clinic nurses employed at general hospitals in A prefecture. Data were collected using a questionnaire created for this research. Questionnaire data indicated that the nurses feel it is difficult to establish relationships with family members who are reluctant to listen to nurses' explanations and accept their advice. The nurses also reported that they could establish closer relationship by spending more time, they could cooperate with other medical professionals, and they had never previously tried to improve their relationships. Although the nurses recognized the importance of working with families to establish partnerships and collaborating with medical professionals, many nurses could not always encourage them to participate. In addition, nurses working in several departments in addition to pediatrics reported feeling that, in comparison to nurses working only in pediatrics, their knowledge and skills were limited in caring for chronically ill children. In conclusion, the present results suggest that it is necessary to establish an effective employment system to ensure that nurses work in only one department and to provide opportunities for nurses to receive education regarding patient support. |
