| 内容記述 | 本研究は,長野県の病院に働く看護職者の抗がん剤曝露予防策の実施状況を明らかにし.看護管理者(以 下，管理者)と看護職者の職業性曝露に対する意識や行動について比較，考察することを目的とした．調査は,郵送で自記式質問紙法とした．対象は,承諾の得られた46施設の管理者549名,看護師1967名である. 2546名に配布し，回収数1265名，回収率49.7%,有効回答率92.3%であった．看護職が調剤に関与している施設は全体の25％であった．５段階で評定を求めた知識に関する項目では曝露の経路や形凱曝露した際の対応などで，１％水準で管理者が有意に高かった(Mann-Whitney Ｕ).予防策実施率では,ディスポ製品の整備/使用， 防護具の整備/使用などで，１％水準で管理者が有意に高かった(が検定)，両者とも実施率が低かった項目は， 健康診断の実施，安全キャビネットの整備，実地訓練であった．これらの結果から，長野県では職業性曝露予防に関する看護職者の知識や環境整備は不十分で，二次的な曝露の危険性が高いことが示唆された， | |||||||||||

| 内容記述 | The purpose of this study was to clarify the present situation of nurses and nurse administrators in Nagano prefecture regarding prevention of occupational exposure to antineoplastic drugs. Their knowledge of and preventive behaviors for this issue were surveyed by a self-rating questionnaire mailed to 549 nurse administrators and 1,997 nurses of 46 hospitals. They were asked to rate their knowledge about prevention of exposure to antineoplastic drugs using five-point scales and answer questions about implementation (nurse administrators' preparation or nurses' use) of preventive measures. A total of 1,265 (49.7%) of them responded and 1,167 (92.3%) respondents(311 nurse administrators and 856 nurse) provided valid data. Mann-Whitney's U statistic was used for comparing the knowledge ratings and chi-square was used for comparing the rates of implementation of preventive measures. Nurses were involved in dispensing antineoplastic drugs in 25% of the 46 hospitals. The administrators rated their knowledge about handling antineoplastic drugs significantly higher than the nurses did (p<.001). Regarding implementation of preventive measures such as disposable products and personal protective equipment, the administrators tended to provide a higher rate of implementation than the nurses. Both the nurses and the nurse administrators indicated low rates of implementation regarding health checks, a safety cabinet, and on-the-job training. The results suggest that the knowledge and measures for prevention of occupational exposure to antineoplastic drugs among nurses in Nagano prefecture are insufficient and hence they are at a high risk of secondary exposure. | |||||||||||
