長野県の訪問看護師の現任教育の現状と学習ニーズを把握するため，県内の訪問看護ステーションに 勤務する管理者を対象に調査を行い，訪問看護師の学習ニーズについて検討した．管理者80名の回答を分析した結果，職場内研修は多くのステーションで行われていたが，職場外研修については，受講することが困難な状況であることが明らかになった．管理者は，初任期には訪問看護の基礎を学ぶこと，中堅期以降にはより専門性の高いニーズに応えるための知識・技術を学ぶとともに，組織全体の視点も学ぶ必要があると捉えていた．管理者自身もまた組織のリーダーとしての研修が必要であるとともに，専門的知識や技術を学ぶ必要があると捉えていた．これらのことから，今後，特に初任期には訪問看護・在宅看護の基本を学び直す機会が保障されることが望ましいと考える．また，訪問看護の実践を通して学ぶ内容（ＯＪＴ）の意味づけや，二次医療圏の範囲で研修を行うなどの学習の機会の検討が必要である． |

The purpose of this study is to clarify the present situation of in-service training and learning needs for home-visit nurses in Nagano Prefecture, We questionnaired nurse managers of home-visit nurse stations and surveyed the learning needs for visiting nurses. Through the analysis of answers from 80 managers, it was found that they feel it is important for new staff members to relearn the fundamental theories and skills of home-visit nursing, and for experienced staff to improve their technical knowledge and to elevate their viewpoints to the organization holistic viewpoints. Moreover, managers themselves feel they need leadership training and further improving their technical knowledge. Among other things, novice home-visit nurses need to relearn the fundamentals theories and skills of home-visit nursing and home-care service. For these purposes, it is necessary to theorize the contents of OJT and to offer theory-laden training programs implemented in the field of the second medical care. |
