Seeing doctors, alcohol-aversion drugs, and self-help groups (SHG) are regarded as the triad methods for alcohol abstinence. The factors affecting alcohol abstinence and SHG attendance have been little studied in Japan and are likely to vary according to human-geographic conditions. Two thirds of Japan is semimountainous areas where life is more community-oriented than in urban areas. [Purpose] The present study intended to identify personal conditions, environmental variables, and coping styles related to alcohol abstinence and SHG attendance to find appropriate means to support people with alcohol dependence living in communities in semi-mountainous areas and adjacent provincial cities in Japan. [Method] The first author interviewed 4 male SHG members keeping alcohol abstinence for more than 3 years. The candidates for participants were selected to maximize variability in their living environment (semi-mountainous or urban) and abstinence experiences (with or without failure of abstinence) . The interviews were IC recorded and transcribed verbatim within three days after the interview and summarized by a content analysis regarding meaning units. [Results and Discussion] The results showed that alcohol abstinence was related to 1) denial / acceptance of own alcoholism, 2) family support / codependence, 3) denial of and support by relatives and friends, 4 ) self disclosure to the community, 5) support for keeping employment, 6) difficulty in establishing human relationship, 7) SHG attendance, 8) tie with the hospital, and 9) nurse's understanding.
